- Insurance Information: Have your insurance details ready, including the insurer’s name, mailing address, group/policy/contract number, and your ID/certificate number.
- Patient History Form: You will be asked to complete a new Patient History Form upon arrival to ensure all records are up to date.
- IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the consultation visit.
During Your Appointment
- Comprehensive Assessment: Your initial appointment includes a thorough assessment of your dental needs and concerns, a review of your medical history, a dental examination by Dr. Yaseen, and a cleaning by the dental hygienist.
- Consultation: Dr. Yaseen will explain your diagnosis and treatment options. In some cases, treatment may be administered on the same day.
